    1. I certify that the facts in this application and any supporting documents, (i.e., resumes, certificates, etc.), are true to the best of my knowledge. I understand that my falsification, omission, or misstatement of information on this application or at any time during the employment application process may result in refusal to hire or, if hired, termination of employment. I authorize the Company to conduct a complete background investigation to determine my suitability for employment and to use any information lawfully obtained for any employment purpose permitted under law. This investigation may include checking with the schools and employers identified above, reviewing criminal and driving records, performing credit checks, and verifying any other information about me. I therefore authorize any appropriate person or entity to disclose to the Company any requested information, including a transcript, final grades, courses attempted and/or completed, and degree(s) earned, and the fact of my past employment, dates of employment, titles, positions, wages, and the reasons for and circumstances surrounding my separation. I authorize the Company to conduct this investigation for up to one year from the date below. Further, I agree to release, hold harmless, and waive any claims I may have against the Company and any and all schools, former employers, consumer reporting agencies and other persons or entities providing information to the Company (whether or not identified above) for any loss or injury I may sustain as a result of any disclosure related to this investigation.
    4. I understand that if hired, I will be required to comply with all Company policies, including the Company’s Confidentiality Pol icy requiring, among other things, that any proprietary information to which I may be exposed in the course of my employment be kept strictly confidential. I further understand that Company policies and benefits, other than the Company’s employment at‐will policy, may be changed, modified, or discontinued at the sole option of the Company, with or without prior notice.
    5. I understand that this application is current for only 60 days. At the conclusion of that time, if I have not heard from the Company and still wish to be considered for employment, I understand it will be necessary for me to complete a new application.
